SLaM - Improving Access to Psychological Therapies in Croydon, Lambeth, Lewisham and Southwark
To begin with clinicians at Croydon Talking Therapies (IAPT) will speak with you about your current difficulties and will carefully consider if the service is right for you. Initial brief assessments usually take place over the phone and can provide interpreters if needed. If our service is not best suited to meet your needs we will put you in touch with another service that is or we will provide you with information on services so that you can make your own choice.
How to access the service
- Complete the online self-referral form and after completing it please call in on 020 3228 4040 to opt in to the service.
- Call us on 0203 228 4040 to register (please note that telephone lines get very busy during the day).

We will then book you in for a telephone assessment.
Before your telephone assessment, you will need to download and complete this PHQ9 GAD7 Questionnaire. Please ensure the scores are totalled up and you have it with you for your assessment.
