Medical certificates are legal documents and will be issued only if deemed neccessary by the clinicians.
Please note: The system will not let us backdate medical certificates. The start date has to be the date of issue/assessment.
If you are requesting for a sick note for a hospital stay you need to contact them not the GP surgery.
Less than < Seven days off sick
If you're off of work sick for seven days or less, you can complete a SELF CERTIFICATION FORM here!
(Your employer has no legal ground to ask for a medical certificate from the GP and should be providing you with this form from HR).
More than > seven days off sick
If you're off work sick for more than seven days your employer may ask for a Medical Certificate from your GP or hopstial doctor. Please ask for a telephone appointment to assess this request. Medical certificates are legal documents and will be issued only if deemed neccessary by the clinicians.
Repeat medical certificates may be requested via email: firstname.lastname@example.org or the Live Chat function
This will take a minimum of 48 hours.

School & University Absences
We are unable to provide schools and universities with sick notes or letters.
